The double dilemma of compliance and doubtful efficacy of silicone products.

Silicone gel sheeting or cushions and pressure garments are difficult and troublesome, which makes compliance very low not only on exposed areas such as the face where it is not aesthetic.

Regardless of the indication, habitual use of a particular product is a must to ensure results. Far too many people fail to complete or follow suggested usage for over-the counter (OTC), as well as prescription products, as a rule.

Silicon gel sheeting is a soft, self-adhesive sheeting to be used on intact skin for preventing and treating both new and old scars.

The authors of a "Cochrane Review" (*) wanted to find evidence on whether silicon gel sheeting prevents the development of scarring in people with recently healed wounds, and it´s supposed effectiveness in treating established scars. They searched the medical literature for randomized and controlled clinical trials. They found 13 trials with 559 patients, 3 trials were prevention studies, 8 trials looked for a treatment and 2 trials looked at both prevention and treatment strategies. Their conclusion: " Most studies were of poor quality and it is unclear whether silicon gel sheeting helps".

Another study, published in 2006, in Archives of Facial Plastic Surgery: official publication for the American Academy of Facial Plastic and Reconstructive Surgery, Inc. and the International Federation of Facial Plastic Surgery Societies, concludes: "Most treatments for keloidal and hypertrophic scarring offer minimal likelihood of improvement."

OBJECTIVE: This study compares the efficacy of a self-adherent soft silicone dressing with 'left-alone' management of hypertrophic scars using theVancouver Scar Scale. Secondary objectives were to follow photographs of the scars, patients' opinions of the scars, and doctors' and patients' assessments of the overall dressing performance, safety and tolerance.

METHOD: An exploratory open randomised controlled clinical investigation was undertaken on 11 female patients aged 21-43 years with postoperative scars (nine following breast surgery, two following lower abdominal-glutealplasty). Treatment was initiated between two weeks and two months (mean 4.7 weeks) after surgery. Ten patients completed the 12-month investigation; one patient in the treatment group discontinued for personal reasons.

RESULTS: All parameters in the Vancouver Scar Scale improved in both groups, although patients treated with the soft silicone dressing showed greater and more rapid improvements compared with the non-treated patients, while their assessments of the condition of the scar were more favourable. Medical staff rated the overall dressing performance as 'very good' or'good'. One adverse event was reported--local skin irritation at the site of the scar.

CONCLUSION: The results suggest that patients treated with the soft silicone dressing experienced greater and more rapid improvements compared with non-treated patients. These results concur with those of previous studies. The fact that Mepiform is self-adhesive and causes limited damage to the stratum corneum on removal gives it an added value compared with non-adhesive silicone gel dressings.
